My friend's been in and out of hospital for a year, what to expect?

She had a hole in her heart when she was a baby, I know it's not a big deal when your young but she's 27 and it's back. Plus she's prone to strokes, clogged arteries and bruising. Every time she goes to a doctor they schedule more tests to be done and never come to any conclusions. She smokes, a lot. I want to know if I can expect her to get better or will things get worse?
